Intake Process
- Section 1: Getting Started
- Application Source: Applications can be downloaded below, or obtained from Dominique Acevedo via email at dacevedo@chs-adphila.org.
- Section 2: Submission
- Completion: Ensure the application form is completed fully with all required information.
- Submission: Return the completed application form to the Program Social Worker Dominique Acevedo via email dacevedo@chs-adphila.org.
- Section 3: Initial Review and Contact
- Review: The Program Social Worker will review the application form upon receipt.
- Timeline: The social worker will contact the parent or guardian within 72 hours of receiving the application.
- Explanation: During the initial contact, the social worker will explain the program details and expectations for parent engagement.
- Section 4: Scheduling an Interview
- Interest Confirmation: Confirm if the child and parent/guardian are interested in proceeding with the program.
- Interview Setup: If interested, schedule an interview with the Social Worker and the Shop Instructor.
- Interview Details: Provide the parent/guardian with the date, time, and location of the interview.
- Section 5: Interview Process
- Conduct Interview: The Social Worker and Shop Instructor will conduct the interview with the child and parent/guardian.
- Assessment: Assess the child’s suitability for the program based on the interview.
- Section 6: Decision Making
- Acceptance: Determine if the child is accepted into the program based on the interview assessment.
- Notification: Inform the parent/guardian of the decision.
- Accepted: Provide details on the next steps and start date.
- Denied: Explain the reasons for the decision and offer alternative resources if applicable.
- Section 7: Waitlist Management
- Waitlist Placement: If there are no current openings, place the child on a waitlist for the next available session.
- Waitlist Notification: Inform the parent/guardian about the waitlist status and estimated wait time.
- Follow-Up: Maintain regular communication with waitlisted families to update them on their status.
